The Consular Department of the Ministry of Foreign Affairs of Ukraine recommends that all foreigners and stateless persons who intend to apply for a visa must pre-fill out a visa application form and register a visa application at the web-site http://visa.mfa.gov.ua.

Necessary documents:

1) passport document

2) a completed and signed visa application form. http://visa.mfa.gov.ua

3) one color photograph 35 * 45 mm;

4) a valid health insurance policy covering the cost of at least 30,000 euros or equivalent in another currency.

5) Bank account. Documents confirming the availability of sufficient financial support for the period of the planned stay ($ 40 a day) and for returning to the State of origin.

6) Invitation In addition to the above documents, a document confirming the purpose of the trip to Ukraine (depending on the type of visa for which visa application is filed) must also be provided. Such documents can be:

                   For short-term "С" type visas:

- invitation of a legal entity (issued on an official form)

The invitation should contain a number in the Unified State Register of Legal Entities, individuals - entrepreneurs and public formations, registration number, date and signature, surname, name, patronymic of the invited person, information about its date and place of birth, citizenship, details of the passport document , place of residence, purpose of the trip, period of planned visit of Ukraine, number of entries and place of residence in Ukraine, obligation of the legal entity to assume possible expenses related to staying and departure from Ukraine invitations Noah person;

- invitation of an individual (notarized)

Invitation to a citizen of Ukraine or a foreigner or stateless person who is legally residing in Ukraine temporarily or permanently. The invitation must contain the surname, name, patronymic of the individual, details of her passport document and certificates for permanent or temporary residence (for foreigners and stateless persons), address of residence, as well as surname, name, patronymic of the invited person, information about its date and place of birth, citizenship, details of the passport document, place of residence, purpose of the trip, the period of the planned visit to Ukraine, the number of entries and place of residence in Ukraine, the obligation of the individual to assume possible expenses, according to associated with staying and leaving Ukraine from the invited person. To the invitation, copies of the passport document and certificates of permanent or temporary residence (for foreigners and stateless persons) in Ukraine of the host party - an individual;

- invitation of the state body (state institution, state enterprise, state organization);

- certificate of a foreign Ukrainian. The marriage of a foreign Ukrainian and his children submit a document confirming the family relations;

- invitation to health care institution;

- a document confirming the right of ownership of a foreigner or stateless person to real estate in the territory of Ukraine;

- appeal of state bodies of foreign states or international organizations;

- confirmation document that the foreigner or stateless person is a husband, wife, father, mother, child of a citizen of Ukraine;

 Also, other documents, if provided by international treaties of Ukraine.

 For long-term type "D"

- a certified copy of the work permit (employment contract or contract).

- a copy of the decision on granting an immigration permit issued by the LCA;

- invitation to study issued by a higher education institution

- a document confirming the fact of being married to a citizen of Ukraine.

 Also, other documents, if provided by international treaties of Ukraine.

Note: Documents must be legalized, unless otherwise provided by law or an international treaty of Ukraine, and presented together with a certified translation into Ukrainian into the prescribed manner, and in the absence of such an opportunity, to the English language.
